Steps to Obtain a Proclamation

  1. Locate the contact information for the government office to receive the proclamation request. Click here to view the list of all US Governors. State guidelines for obtaining proclamations may be available, so search the site. Many states accept the requests through an online submission or email feature. If specific guidelines are stated, follow those provided, otherwise, follow the procedure outlined here.

  2. Carefully read the sample proclamation and enter specific accurate information for the city or state in which the proclamation is being requested.

  3. Carefully read the sample letter and enter specific information, including contact information. Optionally, it would appropriate to add a personal statement, perhaps a relationship to someone with MSA as a motivation for making this request.

  4. Provide some additional Information on Multiple System Atrophy, such as any pamphlets, websites, or other resources available.  Resources

  5. Submit the materials through the website, by email, or regular mail as appropriate to the state procedures. You may want to send a hard copy via mail in addition to your electronic submission.

  6. Follow-up in two to three weeks if there has been no reply. You may also want to get assistance from your local representative or state Senate/Assembly Health Committees.

  7. Please complete the form at this site listing your name, contact information, location of requested proclamation, and status ā€“ including an update after submission. Our goal is to track progress and try to get all 50 states involved!
